What are the Common Risk Factors for Recurrent Corneal Erosion?
Recurrent Corneal Erosion (RCE) occurs when the outermost layer of the eye (the epithelium) fails to properly attach to the layer beneath it (Bowman's layer). When this loose layer tears away, it leaves highly sensitive corneal nerves exposed. The most common risk factor for developing RCE is a previous injury to the eye, such as a corneal abrasion or ulcer from a fingernail scratch or a tree branch.
However, it is not always caused by trauma. People are also at high risk if they have underlying corneal diseases, specifically corneal dystrophies like Epithelial Basement Membrane Dystrophy (EBMD) or granular corneal dystrophy. In some cases, familial genetic mutations (such as the COL17A1 gene mutation) can cause the condition.
How Can I Prevent Corneal Erosion When Waking Up?
RCE attacks primarily happen at night or the exact moment you wake up, often triggered by your eyelids pulling off the poorly attached corneal cells. To prevent these agonizing mornings, you can adopt a specific pre-bed and waking routine:
Before Bed: Humidify the air in your bedroom, avoid sleeping directly under fans or air vents, and maintain good bodily hydration. Apply a long-lasting or hyperosmotic eye ointment before sleep to strengthen the epithelial structure and keep the eye lubricated.
Upon Waking: Train yourself to wake up with your eyes kept entirely still and closed. Before attempting to open your eyelids, gently press on your closed eyelid with a fingertip to push natural lubricants over the cornea, freeing the lid from the eye's surface. Keep artificial tears within reach on your nightstand so you can squirt them into the inner corner of your closed eye, allowing the liquid to seep in before you fully open your eyes. Avoid sleeping in too late, as your corneas dry out more the longer your eyes remain closed.
What is 'Pain Without Stain' (Ocular Neuropathic Pain)?
If you experience relentless eye burning or a feeling of "paper cuts" but your eye doctor sees absolutely zero damage on your eye's surface, you may have Ocular Neuropathic Pain (also known as corneal neuralgia). Often referred to as "pain without stain" (because diagnostic dyes show no physical injury), this condition is caused by damaged or dysfunctional nerve fibers.
Whether due to excessive UV exposure, dry eye disease, trauma, or central nervous system sensitization, these malfunctioning nerves send false, severe pain signals to the brain.

The "Needle-In-The-Eye" Headache: Sometimes a sudden, terrifying stabbing pain in the eye isn't an eye condition at all. It is medically known as an "ice pick headache" (or ophthalmodynia periodica). This sudden, severe stab lasts only about 3 seconds and disappears as quickly as it arrived.
Surgical Taping is a Real Treatment: For people who suffer from Nocturnal Lagophthalmos (a condition where the eyelids do not close fully during sleep), using medical tape to literally tape the eyes shut at night is a highly recommended and effective way to prevent morning corneal erosions.
Pain So Severe Patients Request Eye Removal: Ocular neuropathic pain is historically so misunderstood and can reach such agonizing levels that patients have actually requested doctors to surgically remove their perfectly healthy-looking eyes just to escape the constant pain.
