The thought of any eye procedure can naturally cause a little fear and anxiety. If your doctor has recommended punctum plugs to treat your dry, stinging, or tired eyes, you might be imagining a complicated surgery and a long recovery time. Fortunately, the reality is the exact opposite!
Inserting punctum plugs is one of the simplest, quickest, and best-tolerated treatment options for dry eye. Here is a step-by-step overview of exactly what to expect, so you can leave your fears at the door.
What to Expect During the Procedure
What it looks like: just a few minutes in a comfortable chair. Forget the operating room and hospital gowns. The entire process of inserting punctum plugs takes only a few minutes per eye. You are fully awake throughout the entire procedure and lie comfortably in the procedure room—just a regular visit to your eye doctor.
A Completely Painless Process
The most common question patients ask is: "Is it painful?" The answer is no. Before the procedure begins, the doctor instills numbing eye drops (such as oxybuprocaine) into your eye, making the surface of the eye and the eyelid margin completely numb. Thanks to these drops, inserting the plug is completely painless.
Once the eye is numb, the doctor uses a small specialized probe or forceps to gently place the rice-grain-sized plug directly into the tear duct opening (usually on the lower eyelid). You might feel only a gentle pressure or a slight stretch on the eyelid, but the plug never touches your eyeball, and the whole process is over in seconds.
Testing with Temporary Plugs First
Doctors do not usually insert permanent plugs right away on the first visit. Instead, they often start with a "trial period" using temporary dissolving collagen plugs. This helps to test whether retaining tears provides the desired relief for your specific eyes.
For this trial period, KSA Vision Clinic may use temporary collagen plugs that dissolve independently in the body within several months. Because they absorb naturally into the body, the doctor usually does not need to remove them.
If your eyes feel significantly better during these months, it proves that this method works for you, and you can later discuss placing a permanent silicone plug with your doctor.
Recovery: No Downtime Required
One of the biggest advantages of punctum plugs is that there is absolutely no recovery time. After the plugs are inserted, you can leave the clinic immediately, and there are no restrictions on driving—you can safely drive yourself home or to work.
In the first few hours after the procedure, the eye may feel a bit strange or even paradoxically a little drier as it adjusts to the new situation. Do not worry—this feeling is completely normal and passes on its own very quickly.
The only "rule" after insertion is to be careful with your hands. You should avoid heavy eye rubbing or intense crying, as strong physical contact can dislodge the plug in the lower eyelid and cause it to fall out. If a plug accidentally falls out, it is not an emergency; you simply need to go back to the doctor so they can easily put a new plug in its place. If dryness keeps returning, a dry-eye diagnostic visit helps decide whether plugs, drops, lid care, or another treatment path makes most sense.
