Eye Misalignment. A Parent's Treatment Guide

Childhood strabismus is usually treated first with glasses or patching, not surgery. Eye muscle surgery is reserved for misalignment that persists despite this.

Eye Misalignment. A Parent's Treatment Guide

From corrective glasses to gentle surgery — how we restore visual teamwork.

When parents discover that their child has strabismus or an eye coordination problem, their first reaction is often an anxious one: "Does this mean my child will need surgery?"

The reassuring truth is that surgery is rarely the first step. Modern pediatric eye care offers a gentle, multi-stage roadmap of effective non-invasive treatments designed to align the eyes naturally and help the brain learn to use both eyes together.

Whether your child is a toddler or an elementary school student, understanding these treatment steps empowers you to support their journey toward healthy, comfortable visual alignment.

Spotting the Subtle Signs at Home

Children rarely complain about eye coordination problems because they assume everyone sees the world exactly the way they do. However, parents and teachers can look out for subtle behavioral cues:

  • Squinting or closing one eye when stepping into bright sunlight.
  • Tilting or turning the head to one side when trying to focus on a toy, book, or screen.
  • Frequent eye rubbing or complaining of tired eyes after reading or drawing.
  • Losing their place while reading or holding books unusually close to their face.
  • Difficulty catching balls or appearing unusually clumsy during playground games.

The Non-Surgical Toolbox: The First Lines of Care

Treatment plans are always tailored to the child's age, visual acuity, and degree of misalignment. Specialists typically begin with non-invasive solutions:

1. Corrective Glasses (Prilliravi)

In many cases, inward eye turning (esotropia) is triggered by uncorrected farsightedness (hyperopia). Because the child has to strain their focusing muscles extra hard to see clearly, their eyes automatically pull inward toward the nose (accommodative esotropia). Putting on the right prescription glasses relaxes that strain, allowing the eyes to straighten naturally.

2. Prism Lenses (Prismad)

Prism lenses are special spectacle lenses that gently bend incoming light rays. Instead of forcing tired eye muscles to pull the eye straight, the prism shifts the light beam so it lands directly on the center of the retina. This reduces strain, eliminates double vision, and helps the brain practice merging images comfortably.

3. Patching and Orthoptic Therapy (Katmisravi ja silmaharjutused)

If one eye has become weaker due to suppression, orthoptists may recommend occlusion therapy (wearing a soft eye patch over the stronger eye for a set period each day). This encourages the brain to reconnect with and strengthen the weaker eye. Combined with targeted orthoptic exercises, patching builds strong, balanced visual pathways.

When Is Eye Muscle Surgery Considered?

If glasses, prisms, and orthoptic exercises cannot fully correct the physical misalignment of the eyes, an eye doctor may discuss strabismus surgery (silmalihaste operatsioon).

How it works: Strabismus surgery does not operate inside the eyeball itself or alter visual sharpness directly. Instead, the surgeon works on the tiny extraocular muscles attached to the outer surface of the eye. By gently repositioning or adjusting the tension of a muscle, the surgeon restores physical alignment.

The Goal: Surgery brings the visual axes into parallel alignment so the eyes can point in the same direction effortlessly, providing the best foundation for long-term 3D vision and comfortable eye movements.
