When we talk about age-related macular degeneration (AMD), we are talking about a condition that has two very different personalities. Eye specialists divide AMD into two forms: Dry and Wet.
While both affect the macula—the tiny powerhouse at the center of your retina—they behave in entirely different ways. One is a slow, gradual marathon; the other is an aggressive, sudden sprint. Understanding the difference between these two types, and knowing what warning signs to watch out for, can literally mean the difference between preserving your vision and losing your independence.

Dry AMD: The Slow-Moving Marathon
Dry AMD is by far the more common form, accounting for about 85% to 90% of all diagnosed cases.
Dry AMD is a gradual process. Over several years, the retinal cells in your macula slowly wear down and die off, and the body cannot renew them. It typically progresses through three stages:
- Early: Small yellow deposits (drusen) form beneath the retina, but vision remains completely normal.
- Intermediate: Drusen grow larger and pigment changes occur. You might begin to struggle with reading in dim light or notice that colors look slightly faded.
- Late (Geographic Atrophy): Large patches of cells in the macula completely waste away. This creates permanent dark or blank spots in your central vision.

Wet AMD: The Aggressive Sprint

The Damage: This excess VEGF commands the body to grow new blood vessels underneath the macula. However, these new vessels are abnormal, extremely fragile, and poorly constructed.
The Leak: Like a cheap, leaking garden hose, these vessels leak blood, lipids, and fluid directly into the delicate layers of the macula. This fluid causes the macula to swell up and lift away from its normal position.
In the short term, this fluid causes sudden, painless visual distortion. Straight lines suddenly look wavy, door frames appear warped, and objects can look smaller or larger than they actually are.
If left untreated, within just a few days or weeks, this pooling fluid and blood will cause permanent scarring of the macular tissue. Once scar tissue forms, the damage is irreversible, and central vision is lost permanently.
The Visual Red Flags: The Amsler Grid and Daily Life
Because wet AMD can strike suddenly, patients with dry AMD must continuously monitor their eyes. The conversion from dry to wet AMD can happen at any time.
The most powerful weapon you have at home is a simple tool called the Amsler Grid—a grid of straight black lines on a white background with a dot in the center. By looking at this grid (covering one eye at a time), you can instantly spot changes:
- The Wavy Warning: If the straight lines on the grid suddenly look wavy, bent, or distorted, this is a major warning sign of wet AMD.
- The Gray Spot: A sudden, gray-appearing smudge or dark blurry spot in the center of the grid indicates fluid is accumulating.
- The Missing Grid: If parts of the grid disappear entirely, it means retinal cells are actively struggling.
Outside of the grid, you might notice these red flags in daily life: a telephone pole that looks bent, a straight line of text on your phone that suddenly dips, or a sudden blank spot when looking at a loved one's face.
The Good News: How Modern Medicine Fights Wet AMD
A decade ago, wet AMD was considered an untreatable, one-way ticket to blindness. Today, the outlook is incredibly positive thanks to a class of medications called anti-VEGF drugs.
These groundbreaking drugs—such as Lucentis, Eylea, Vabysmo, and Avastin—are administered as microscopic injections directly into the eye. They work by binding to the excess VEGF protein, neutralizing it, and effectively "turning off" the signal that causes blood vessels to grow and leak.
