One Principle, Unchanged for Twenty Years

When Dr Ants Haavel founded KSA Silmakeskus in 2005, the clinic's philosophy crystallised around a single rule: never do more to an eye than necessary.

That principle didn't come from textbooks or conference slides. It came from the operating theatre — from years of seeing eyes that had been over-treated, cut too aggressively. Eyes that had been handled as a problem to solve, not as the most precious sensory organ a person owns.

"I saw patients returning with corneal ectasia," Dr Haavel recalls. "That's a condition glasses and contact lenses can't fix. Someone came to escape the burden of spectacles and left with a complication far more serious. I couldn't accept that trade-off."

Why the Flapless Path

When entering refractive surgery, Haavel faced a choice that was unconventional at the time. LASIK advocates promised speed: quick recovery, good vision by next day. Surface ablation meant slower healing, more patient responsibility, more patience required.

The research was telling. Studies show LASIK preserves only 2–28% of the cornea's original strength. The risk of ectasia after LASIK is roughly 4.5 times higher than with surface ablation. The flapless method — Flow3 — creates no irreversible cut in the cornea. Corneal biomechanics remain intact.

That wasn't just safer on paper. In clinical practice over two decades, the difference became unmistakable.

"That safety record across 55,000 procedures is one of the most precise measures of safety in refractive surgery in the Baltics," Dr Haavel says. "It doesn't happen by chance. It comes from three things working together: rigorous pre-operative screening, careful patient selection, and the flapless method itself."

Pre-operative assessment at KSA takes 60 minutes and includes thousands of measurements — corneal topography, thickness mapping, biomechanical indices. Roughly 15% of patients who arrive wanting surgery leave with the answer "no." The cornea is too thin. The topography irregular. The expectations don't match reality. That honesty matters as much as technical precision.

It's the difference between a clinic that sells procedures and one that solves problems.

What Twenty Years Has Taught

Patience is part of the outcome. With flapless surgery, healing takes time. The first week is blurry. Stability comes over two to three months, sometimes longer. That's the price of preserving corneal integrity — and it's a price worth paying.

There's another advantage, too. In Dr Haavel's clinical experience, presbyopia (age-related reading vision loss) develops approximately five years later in patients treated with flapless methods compared to corneal-cutting approaches. That means a longer period of visual freedom in daily life.
